1）投标人必须满足《中华人民共和国政府采购法》二十二条之规定：

2、拟参加本项目投标的潜在供应商须在黑龙江省内政府采购网注册登记并经黑龙江省政府采购管理办公室审核通过；

3、投标人必须满足以下商务条款：

（1）对于国外、境外投标人，根据该国（地区）的法律在经营所在地注册的有关证件；

（2）制造商应具备《医疗器械生产企业许可证》、经销商应具备《医疗器械经营企业许可证》，经销商应具备制造商的授权或代理经销协议；

     投标产品应具备《医疗器械注册证》和《医疗器械注册登记表》；

     进口产品应具备《进口医疗器械注册证》（如注册证到期，需提供海关等相关证明，证明该设备是在注册证有效期内生产完成的，否则不允许投标）；

（3）如国内代理商投标，国内代理商必须是经过制造商正式授权的合法代理商（提供代理证明）；

4、是否接受联合体投标：不允许；

5、拟参加本项目潜在供应商未被列入重大税收违法案件当事人名单，核查路径：<国家税务总局>查询网址：http://hd.chinatax.gov.cn/；

拟参加本项目潜在供应商、供应商法定代表人及授权委托人未被司法机关列入失信被执行人名单，核查路径：<信用中国>查询网址：http://www.creditchina.gov.cn/；

   拟参加本项目潜在供应商未在政府采购活动中有严重违法失信行为记录，核查路径：<中国政府采购网>查询网址：http://www.ccgp.gov.cn/；

如存在不良信用记录，其资格将被当场取消；

1 ) The bidder must satisfy the provisions of Article 22 of the Government Procurement Law of the People's Republic of China:
2. Potential suppliers who intend to participate in the bidding of this project must be registered in the government procurement network in Heilongjiang Province and approved by the government procurement management office in Heilongjiang Province.
3. The bidder must meet the following commercial terms:
( 1 ) For foreign and overseas bidders, relevant certificates registered in the place of operation according to the laws of the country ( region );
( 2 )The manufacturer should have the "Medical Device Manufacturing Enterprise License", the distributor should have the "Medical Device Operating Enterprise License", and the distributor should have the manufacturer's authorization or agency distribution agreement;

 The bidding products should have the "Medical Device Registration Certificate" and the "Medical Device Registration Form";

 Imported products should have the "Imported Medical Device Registration Certificate" (if the registration certificate expires, relevant certificates such as customs must be provided to prove that the equipment was produced within the validity period of the registration certificate, otherwise bidding is not allowed);

（3）If a domestic agent bids, the domestic agent must be a legal agent duly authorized by the manufacturer ( providing agency certificate );
4. Whether to accept the consortium bid: Not allowed;
5. Potential suppliers who intend to participate in this project are not included in the list of parties to major tax violation cases. Check path: <State Administration of Taxation> query website: http://hd.chinatax.gov.cn/;
