河南省机电设备国际招标有限公司受招标人委托对下列产品及服务进行国际公开竞争性招标。 本次招标采用传统招标方式，现邀请合格投标人参加投标。

投标人应具备的资格或业绩：投标人应符合： （一）具有独立承担民事责任的能力； （二）具有良好的商业信誉和健全的财务会计制度； （三）具有履行合同所必需的设备和专业技术能力； （四）有依法缴纳税收和社会保障资金的良好记录； （五）参加政府采购活动前三年内，在经营活动中没有重大违法记录；3.2具有生产厂家针对本项目所投设备的授权书；3.3提供投标产品的技术白皮书或检测报告； 3.4提供法定代表人授权书、法人身份证复印件、被授权人身份证； 3.5提供医疗器械经营企业许可证副本（且在经营范围内）； 3.6提供医疗器械注册证复印件； 3.7提供销售合同及中标通知书3份；3.8同一个品牌只允许一个生产商或一个代理商参加投标； 3.9本项目不接受联合体投标。 投标人报名时需提供上述报名资料原件备查、复印件一套加盖公章。
